Vessels, Bladder & Uterus

Three more rapidly dividing targets โ€” one of them stops your period entirely.

Three New Targets

Flip Each Card

Front = what fails. Back = the mechanism.

Vascular
๐Ÿฉธ
Endothelium
Blood vessel lining
ATP needed forVessel wall repair + collagen
Fails becauseCan't replace worn cells
Resultfragile ยท cracked ยท leaky
โš  Petechiae, easy bruising, poor healing
tap to flip โ†’
Why It Happens
Normal
Endothelial cells divide constantly to repair micro-damage from shear stressPulsatile blood flow creates mechanical shear force on the endothelium. Cells are continuously damaged and replaced โ€” high turnover, high ATP demand. from blood flow. Collagen synthesis holds the wall together.
No ATP
Collagen synthesis stops. Cell replacement slows. Micro-tears don't heal โ†’ wall thins โ†’ blood leaks out โ†’ bruising, petechiae.
Board link
Same mechanism as skin โ€” epithelial cell line that can't renew. Just a different surface.
Urinary
๐Ÿ’ง
Bladder Urothelium
Transitional epithelium lining
ATP needed forBarrier cell renewal
Fails becauseCan't replace worn cells
Resultthin ยท permeable ยท irritated
โš  Urine contacts submucosa โ†’ pain, urgency
tap to flip โ†’
Why It Happens
Normal
Transitional epithelium forms a chemical seal between concentrated, acidic urine and the bladder wall. Needs constant renewal โ€” urine is chemically harsh on the cells.
No ATP
New cells can't form. Old cells wear down. The seal breaks โ†’ urine penetrates into the submucosa โ†’ irritation, urgency, pain.
Board link
Same story as GI mucosa โ€” epithelial barrier fails when cells can't renew. Different organ, identical mechanism.
Reproductive
๐ŸŒธ
Endometrium
Uterine lining
ATP needed forMonthly full rebuild
Fails becauseCan't replicate monthly
Resultno period ยท no implantation
๐Ÿ”‘ Secondary amenorrhea โ†’ sterility
tap to flip โ†’
Why It Happens
Normal
The endometrium completely demolishes and rebuilds itself every ~28 days. One of the most ATP-hungry processes in the body โ€” done monthly, every month.
No ATP
Endometrial cells can't replicate. No new lining = secondary amenorrhea. No lining = no nourishment for a fertilized egg = sterility.
Key distinction
Secondary โ€” she had periods before. Energy depletion stopped them. This differentiates from structural/developmental causes.

The Uterus Cascade

Build the Chain

Predict each step before it unlocks.

1
ATP drops in the body. Endometrial cells need ATP specifically to do what?
โœ“ The endometrium is one of the most rapidly dividing tissues in the body. Complete demolition and reconstruction monthly โ€” all of that cell division costs enormous ATP.
2
Endometrial cells can't replicate. The patient previously had normal periods. What's the clinical term?
โœ“ Secondary. She HAD periods โ€” this is key for boards. Low energy state suppresses a previously functioning system. Primary = never started. Secondary = started, then stopped. Don't mix these.
3
No endometrial lining is being rebuilt. Ovulation still occurs and a fertilized egg reaches the uterus. What happens?
โœ“ The endometrium is the nutritive sourceBefore placental circulation is established (~weeks 1-2), the decidualized endometrium directly nourishes the implanting embryo via glandular secretions. No lining = no food = embryo doesn't survive. for the early embryo. No lining, no nourishment โ€” implantation fails and the embryo cannot survive.
โœ“
Cascade complete.
Low energy state โ†’ endometrial cells can't replicate โ†’ secondary amenorrhea โ†’ no endometrial nourishment โ†’ sterility ๐Ÿ”‘No fuel, no future. The uterus rebuilds like demolishing and reconstructing a house every 28 days. Cut the power and construction halts. No house = no tenant can move in.
๐Ÿšจ
Board Trap: Primary vs Secondary Amenorrhea
Primary amenorrhea = periods never started. Structural, genetic, or developmental cause (Turner syndrome, absent uterus, androgen insensitivity).

Secondary amenorrhea = periods existed, then stopped. Low energy state โ†’ ATP depletion โ†’ endometrial cells can't replicate = secondary. The word "previously had normal cycles" in a vignette = secondary.
