Why It Happens
Normal
Endothelial cells divide constantly to repair micro-damage from shear stressPulsatile blood flow creates mechanical shear force on the endothelium. Cells are continuously damaged and replaced — high turnover, high ATP demand. from blood flow. Collagen synthesis holds the wall together.

Why It Happens
Normal
Transitional epithelium forms a chemical seal between concentrated, acidic urine and the bladder wall. Needs constant renewal — urine is chemically harsh on the cells.
No ATP
New cells can't form. Old cells wear down. The seal breaks → urine penetrates into the submucosa → irritation, urgency, pain.

Why It Happens
Normal
The endometrium completely demolishes and rebuilds itself every ~28 days. One of the most ATP-hungry processes in the body — done monthly, every month.
No ATP
Endometrial cells can't replicate. No new lining = secondary amenorrhea. No lining = no nourishment for a fertilized egg = sterility.
Key distinction
Secondary — she had periods before. Energy depletion stopped them. This differentiates from structural/developmental causes.

Board Trap: Primary vs Secondary Amenorrhea
Primary amenorrhea = periods never started. Structural, genetic, or developmental cause (Turner syndrome, absent uterus, androgen insensitivity).

Secondary amenorrhea = periods existed, then stopped. Low energy state → ATP depletion → endometrial cells can't replicate = secondary. The word "previously had normal cycles" in a vignette = secondary.
